Expert neuro rehabilitation and complex care for people with acquired brain injuries or long-term neurological conditions

QEF’s Care and Rehabilitation Centre is a Level 2B Community Neuro Rehabilitation Centre, providing specialist neuro rehabilitation and complex neurological care for up to 48 resident clients.

Our expert teams support people to regain core skills following an acquired brain injury, stroke or incomplete spinal injury.  We also support people with long term neurological conditions to optimise their potential, manage a disability and prevent secondary complications.

Referrals are through the NHS, personal injury solicitors or case managers, ands we support adults with physical, cognitive and/or communication disabilities which may also include mild to moderate behavioural problems.

We work closely with families and everyone involved with an individual’s long-term rehabilitation, helping them with the transition out of our service. Our services are also available on a day and outpatient basis.

Direct Client Pathways

QEF’s Care and Rehabilitation Centre provides 4 distinct client pathways:

A client’s experience of our service will vary slightly depending on which pathway they are on. Length of stay is determined by the funding authority and agreed multi-disciplinary outcome goals:
– Client journey for Neuro Rehabilitation
– Client journey for General Rehabilitation
– Client journey for Complex Neurological Care

Admission Criteria

We assess each person’s requirements prior to admission and put together client-centred care and rehabilitation plans.

We accept clients with agreed funding who meet the following criteria:
– Males and females with Acquired Brain Injury, neurological illness, stroke or incomplete spinal injury
– Aged from 18+
– Medically stable
– Have the potential to benefit from and the ability to participate in rehabilitation programmes

Measurable Outcomes

We measure and record the progress of every client; comparing their capabilities on arrival to those on discharge.

Working with the NHS 

We work with NHS acute and primary care trusts across the South of England. Working with the Early Supported Discharge, Community and Reablement teams as required, we then facilitate a client’s discharge home or to another suitable destination to continue their rehabilitation.

East Surrey Place has been working in collaboration with QEF for a number of years in supporting our local residents in their rehabilitation and recovery journey following a stroke. Over the years, our partnership is built on mutual desire to support continuity of rehabilitation from East Surrey hospital and ongoing care by our Early Support Discharge team in the Community. This partnership has been further strengthened by QEF offering us some extra rehabilitation beds for patients leaving hospital.”

Our multi-disciplinary team

Our  multi-disciplinary team includes a consultant in rehabilitation medicine, GP, specialist therapists, nurses and care staff.

Our Therapy Team include:

  • Physiotherapists use specific handling techniques and a range of specialist equipment to improve posture, flexibility, muscle strength, coordination, functional ability and mobility.
  • Speech and Language therapists aim to improve speech, language, communication and any eating or swallowing problems that may have arisen as a result of brain injury.
  • Occupational therapists use a combination of rehabilitation techniques to assess and support our clients in completing everyday living tasks such as personal care, mobility, domestic activities, and household management.
  • Clinical Psychologists support individuals, couples and families in coping with the impact of acquired disabilities such as stroke or brain injury.

Nursing and Care: Led by our Senior Nurse, we provide 24 hour medical care and supervision, and dedicated support workers.

Recreation and Activities
A wide range of activities help clients develop confidence and encourage participation and exploration of new interests, such as art sessions, discussion and music groups, gardening clubs, quizzes and social events.

Facilities

A spacious, well equipped building that has been designed to meet the needs of QEF’s clients.  Rooms are modern and bright with a comfortable, homely environment.  There are extensive therapy facilities with a large spacious gym and several adaptable therapy rooms. Clients each have their own en-suite bedroom with tracking hoists and some have additional kitchenettes. Each room is supported by innovative assistive technology and a nurse call system.
